This is Me

this image shows the this is me leaflet from the Alzheimers disease society“This is Me” is a form designed to help healthcare professionals care for someone with dementia should they need care in a new environment such as clinic or hospital. This form details likes and dislikes regarding food etc. and encourages person centred care. There have been some discussions recently regarding those living in a residential environment being admitted to hospital and their ” This is Me” form being left behind.

It is a good idea to fill one of these forms out for someone you care for if they have dementia where ever they live  and placing a copy in a pre-packed hospital bag in case of an emergency admission.

To make it stronger and also waterproof it may be worth laminating it.
