“This is me” – communication tool

The “This is me” leaflet is designed as a communication tool for anyone with Dementia who receives care in any setting;

  • At home
  • In hospital
  • In a respite home
  • In a care home.

Originally developed for people with dementia who were going into hospital it is a  simple and practical tool that people with dementia can use to tell staff in any environment about their

  • needs,
  • preferences,
  • likes,
  • dislikes
  • interests.

Enabling health and social care professionals to recognise the person as an individual and deliver person-centred care specifically to that person’s needs, “This is me” reduces stress for the person with dementia and their carer.

It can also help to prevent issues with communication and serious conditions such as malnutrition and dehydration.

This is me was first developed by the Northumberland Acute Care and Dementia Group and is supported by the Royal College of Nursing.
